Office Hours
8:30 AM to 4:30 PM Monday through Friday.
Consultation
For your safety and to optimize your results, we require a consultation with Dr. Shibayama prior to scheduling any procedures. Consultations are free. As each procedure is customized for your individual needs, we are unable to provide an estimate on fees until after your consultation appointment.
Payment
Cosmetic procedures are not covered by insurance and payment in full is due at the time of service. For your convenience, we accept Mastercard, Visa, ATM, cash, and personal checks. Financing is also available.
Rescheduling
If you need to cancel or reschedule this appointment, we ask that you contact our office at least 24 hours in advance. Patients scheduled for a procedure should provide at least 48 hours notice. There is a $100 fee for all scheduled procedures cancelled with less than 24 hours notice.
Children Under 18
Children under 18 years of age must be accompanied by a parent or legal guardian or present a signed permission slip in order to receive treatment of any kind.
